Services

  • Access to a physician, immunizations, laboratory and x-ray services, family planning services
  • Annual physical examination (provided once per year)
  • Medical screening: Blood pressure, blood sugar, and cholesterol screening
  • Immunizations
  • Prescriptions service: Includes up to 3 prescription drugs per month
  • Inpatient Hospital Services: Must be medically necessary and provided in an acute care hospital
  • Outpatient Hospital Services: Must be medically necessary and provided in an acute care hospital

  • Fill out application (good health card), assistance is needs based
  • Photo ID and Social Security Card

  • Psychiatric Services: once per month, required appointment and referral from primary care provider
  • Orthopedic/Rehabilitation services: once per month, required referral from primary care provider
  • Physical Therapy & Occupational Therapy Services: Twice per month, required appointment and referral from primary care provider
  • Acute Care Medical Service: available only to patients with no insurance, Tuesday and Thursday from 5pm to 7pm, Sign in starts at 4:30pm (first come first served)
  • Ear, Nose , Nose and Throat: once per month, required appointment and referral from primary care provider
  • Pulmonology Services: once per month, require appointment and referral from primary care provider
  • Chiropractic: 2 Tuesdays per month, must sign in by 5:20pm, first come first served
  • Vision: Every Tuesday 5-7pm, sign in starts at 4:30 . First come first served.
  • TB Testing: Every Tuesday 5-7pm, read on Thursday 5-7pm

Requirements

  • Patients get signed in beginning at 4pm every Tuesday and Thursday evening, first come, first served. No immunizations, STD testing, labs, x-rays, or disability forms.
  • Only for patients without Medicare, Medicaid, or any other form of insurance and who are NOT established with FCH.
